The Health Information Management (HIM) Department is committed to the quality, confidentiality and security of health information. To request medical records, contact an HIM associate during regular business hours Monday through Friday. Our associates will assist you in obtaining copies of your information. By calling ahead to request the information, you may avoid unnecessary waiting.

For continuity of care purposes, records can be sent directly to the healthcare provider you designate which eliminates the need for you to make a trip to our office.

Obtaining Records

To obtain records for any other reason, or for someone other than your self, we require a valid photo ID and a right to access from the patient or individual allowed to act as a personal representative for the patient for the release of protected health information. A right to access form may be obtained from our HIM Department or it can be downloaded by clicking below on the section entitled Right to Access Form.

After the form is completed and signed, deliver it directly to the HIM Department or return it via mail with a copy of your photo ID to:

Your records will be sent to the mailing address or e-mail address you provide. You may also pick your records up in our department after presentation of your ID. Medical information will not be released over the telephone. You may also visit our patient portal to view your medical records. Upon discharge from OCH, you will be given a username and password to access electronic medical records for your hospital-based services. HIM staff will also be able to help you re-set your access to the patient portal system if needed.
