Your Registration

Admission and Registration offices are located in both the Front Lobby and the ER Lobby of the hospital. In order to expedite your registration, please bring all paperwork from your physician. Please provide your current insurance information and at least one photo ID. Arrive 10 to 15 minutes ahead of your appointment time to register.

During registration, you will be asked to sign a consent form, at each visit, for permission to perform testing and/or medical treatment and to bill your medical insurance. For outpatient surgery and overnight patients, you will be provided an Identification bracelet displaying your name, birth date, and your physicians’ name. If required, a drug allergy bracelet will be placed on your wrist.

After registration, you will be escorted to the department where you are receiving services. Hospital personnel will interview you by asking general health questions. Federal regulations require admissions personnel to ask you questions regarding the presence of an advance directive. An advance directive does include whether or not you have a living will or durable power of attorney with a health clause. Information on advance directives is provided to every patient upon admission, along with other valuable information.

The admissions staff will happily answer any questions you may have concerning our hospital and your stay with us. We will do our very best to make you feel safe and secure during your visit at Ohio County Healthcare.
